- As far as document requirement is concerned then first, we need the original birth certificate. We need the applicant’s passport. Parents’ marriage certificate is also needed. The application needs to be submitted to the MOHAP online portal. The e-service of MOHAP is available. The existing birth certificate and the amendments needed must be submitted. Make sure that submitted documents should be notarized.
- For the Arabic version only, the fee is AED 65 and for English and Arabic versions, the fee is AED 130. Once the documents are submitted on MOHAP e-portal then follow-up is required.
